Akwa GGZ and my data
Your permission is required to improve mental health care. Are you in?
In The Netherlands, yearly more than 1 milion people need help from mental health care (ggz). For all these people, it is important that practitioners offer the best possible care. They do this by working together on a regular basis and be comparing the results of the treatments, based on questionnaires that you and others fill in. This way, practitioners know which approach is effective and which approach can be improved. To improve care, your data as well are required. We only use these data when you have given your explicit consent. How that works? You will read that below.
Your data are very important
As a part of your treatment, you or your legal representative fill in questionnaires together with your practitioner. With the collected data your therapist can determine if your complaints change over time and if you are doing better. When multiple practitioners gather and compare this information, they learn from each other and work together on improving the quality of care.
Which data do we use?
- The answers to the questionnaires you fill in.
- Data about your complaints for which you are being treated.
- Data about your practitioner.
- General information such as your year of birth, gender and educational attainment.
This is how we process your data
- Akwa GGZ and your practitioner abide by legal regulations.
- Your data are pseudonymized by an independent organization. This means that Akwa GGZ does not know whose data they are working with.
- Confidentially dealing with data is necessary. Therefore, Akwa GGZ staff members have signed for duty of confidentiality.
- Information is also used for research to improve care standards.
Your permission? That works as following
- By giving your permission in the Treatment conditions document, you give your practitioner your explicit consent to use your data.
- Your practitioner saves this form in your file.
- You can withdraw your permission via your practitioner at any given time.
- If you do not give your explicit consent, your practitioner will not forward your data. This has no consequences for your treatment and compensation thereof.
Of course, you have a couple of important rights. You can address your therapist on these.
- You can view your data
- You can correct or supplement your data
- You can request removal of your data
- You have the right to ‘be forgotten’. From that moment on, your data will not be used anymore. This can also be done retroactively.
- You can request to stop using your data temporarily. Your data will be put aside.
- You can request for your data to be sent to you or another controler.
You can also file a complaint with the Authority for Personal Data (Autoriteit Persoonsgegevens).

Grip Psychologists uses a clientportal. This is a secured environment where clients can access their data stored in our system. Data that can be found are:
- Referral letter
- Treatment conditions
- Treatment plan
- List of complaints (SQ48)
- Appointment overview
- Communication: sending secured messages to the therapist
In order to secure good use and legibility of information in the portal, the accessibility statement (Toegankelijkheidsverklaring) has been drawn up. This document is currently only available in Dutch.
